Role Description

Oak Street Health takes a team-based approach to providing outstanding patient care. The OSH Team is responsible for delivering excellent, high-touch, primary care, and coordinating the care of our patients throughout the healthcare delivery system.

Registered Nurses (RNs) are an important part of Oak Street Health’s success. High levels of flexibility, energy, attention to detail, and problem solving are required to be successful. You will be expected to build relationships with Oak Street Health patients, manage patient’s care plans, perform proactive phone and in-person outreach to our patients based on their care needs, support Care Team decision making, participate in weekly care team meetings, and generally coordinate all clinical and auxiliary services needed to provide a high quality health care experience from Oak Street Health. The RN is a crucial member of the interdisciplinary care team, who partner to address care gaps, manage chronic conditions and support behavioral, social and community needs. Through the management of acute and subacute transitions, s/he facilitates appropriate interventions, and collaborates with OSH and hospital/SNF teams as needed. S/he is an assertive problem-solver, motivated to support our population who understands the value of timely discharges, low readmissions and efficient resource utilization.

Responsibilities

  • In-center care support and triage for patients, scheduling problem visits or nurse visits when appropriate, offering symptom management, supporting refill requests and answering visit follow-up questions
  • Provide patient education, particularly around chronic conditions; will mostly occur in person, individually or in a group setting
  • Monitoring and coordinating the delivery of the care plan
  • Works in partnership with multiple care teams to provide daily support to patients in transition away from the hospital, advocating for the patient’s best interest, and intervening in the development of a discharge plan
  • Supports care coordination of patients with complex needs, breaking down barriers to discharge, such as securing referrals or home resources
  • Forming relationships with patients and their caregivers to support prevention focused care and ED/ hospital diversion where appropriate
  • Conduct medication and testing authorizations
  • Develop and maintain strong working relationships with hospital staff in assigned region, including planning and facilitating meetings with hospitals and skilled nursing facilities
  • Identify and implement opportunities for efficient workflows, increased partnership and more effective patient care
  • Report out to OSH providers and leadership on admissions, progress and opportunities for future improvement
  • Other duties as assigned

RNs report to the Director of Care Services

Minimum Requirements

  • 2+ years of relevant experience, or equivalent training plus experience, preferably in a primary care office or other outpatient setting
  • Current state license to practice as a Registered Nurse
  • Exceptional communication skills and customer service orientation for a wide audience including patients, caregivers and OSH and hospital providers and staff
  • Comfort with accountability, and able to communicate on accomplishments and challenges
  • A problem-solving orientation, and a flexible and positive attitude
  • A supportive and empathetic attitude toward our patient population of older adults
  • Valid driver's license or ability to travel daily. This role will require travel between facilities in the Chicagoland area
  • CPR Certification
  • EMR experience
  • US work authorization

Ideal Candidates Will Have

  • Nurse Case Management Credentialing (RN-BC) or be a Certified Case Manager (CCM)
  • Experience with older adults
  • Experience in utilization management and working with acute and subacute discharge planning ideal
  • Ability to monitor and evaluate opportunities for cost effective, high quality outcomes
  • Knowledge of referral coordination to community resources
  • Someone who embodies being “Oaky”.

Pay Range

The Typical Pay Range For This Role Is

$60,522.00 - $129,615.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
